I stopped trusting it when I first saw it, and the NL (which runs every 12 minutes during rush hour) had a wait of 55 minutes before the next bus would supposedly arrive. It's like those crosswalk countdowns that reach zero and just sit there for ten seconds before the light turns yellow: they're training us not to trust information.

Once, about 15 years ago, well before NextBus, I waited 45 minutes during the evening rush hour for a bus on the corner of Van Ness and Eddy in San Francisco. There were three separate bus routes (42, 47 or 49) that would have suited me just fine, and all of them were scheduled to run every 6-8 minutes, but none showed up. Unsurprisingly, by the time four or five of them showed up at once, they were all packed.

A situation like that would probably melt the NextBus server.
